Here is what to expect:

• If you have been referred by your primary care doctor or another specialist, they will share your medical records with us and we will contact you to make your appointment.
• If you are making your own appointment, we may ask you basic questions that provide insight into your health history. Please note, some insurance plans may require a referral from a physician. If so we can guide you through this process at the time of your call.
• If you have recently had any diagnostic imaging tests (MRI, X-rays, CAT scan) related to your condition, please share this information when you request an appointment with our office.
