Southeast Health Care Link

Care Link is Epic’s web-based application for connecting organizations to their community affiliates. It provides community users secure access to select patient information in Southeast Health Epic data repository.

Care Link is not an EMR solution. It is a read-only application with some service-oriented features which are configured to facilitate patient care with community practices.

Examples of community users include people outside of Southeast Health who might need to review the clinical and administrative information of patients seen at our organization. Affiliate clinical staff, billers, long-term care facilities, and correctional facilities are examples of Care Link users.

Care Link is Southeast Health’s secure online tool for connecting private and referring physicians, as well as their staff to our electronic health record systems. It allows providers to view information about their patients being cared for at a Southeast Health facility. Care Link offers view-only access to a patients’ information such as hospitalizations, specialist referrals, and real-time clinical data and is compliant with state and federal regulations for the release of Protected Health Information (PHI).

  • The ability to view a patient’s clinical history, including discrete results, reports and images.
  • Event notifications for your patients (e.g., patient’s Emergency Department visit); Provider must be identified on the patient’s registration.
  • Secure messaging to /from Southeast Health staff regarding patient care, including referral requests.

Yes. Using Care Link, you can access clinical data for your patients, regardless of which Southeast Health facility they visit.

With Care Link, you can view your patient’s lab results, diagnostic test results and reports, hospitalization records, procedural information, discharge instructions, progress notes, medications, allergies, problem lists, medical history and more.

  • Staff at our affiliated practices, whose physicians have full Epic access.
  • Referring physicians and their support staff.
  • Post-acute providers, such as nursing homes and long-term care facilities.

There is no cost to the provider associated with using Care Link.

Care Link uses industry-standard encryption technology, ensuring access is only available to those authorized to view patient data.

Users will need access to the internet through Internet Explorer, Chrome, Firefox or Safari.
