At Southeast Health, care goes beyond your hospital stay. From admission, a dedicated case manager, often a nurse or social worker, supports you and your family and helps coordinate your care.
Working with your care team, your case manager serves as your advocate, helping you understand your needs and plan for a safe, smooth transition after your stay so you feel prepared and supported.
Helping You Navigate the Process
From your first day in the hospital through your recovery beyond it, our Case Management team is here to help you move forward with confidence. Case managers participate in interdisciplinary discussions with your care team and meet with patients and families to review goals, answer questions, and support decision making. We are here to:
Our team also includes notary publics who can assist with important documents such as medical power of attorney, guardianship, and healthcare directives when needed.
Planning for What Comes Next
Discharge planning begins early in your hospital stay because good planning leads to better outcomes and a smoother transition home or to the next level of care.
Our Case Management team works with you to explore your options and coordinate the services you may need after leaving the hospital, such as:
• Home health services or follow-up care
• Rehabilitation services or recovery programs
• Skilled nursing or long-term care facilities if needed
Insurance coverage and medical readiness both play a role in discharge decisions. When post-acute care is recommended, we begin identifying available options early, including facilities that meet your needs and accept your insurance.
