At Southeast Health, we understand that your health information is personal and private. Protecting your medical information is an important part of the care we provide.

As part of your treatment, we create and maintain records of the care and services you receive. These records help us deliver high-quality care, coordinate services among your healthcare providers, and comply with applicable legal and regulatory requirements.

Our Notice of Privacy Practices explains how your medical information may be used and shared, as well as the rights you have regarding your health information. We are committed to safeguarding your privacy and handling your information responsibly.

Health Information Exchange (HIE) Participation

Southeast Health currently utilizes Care Everywhere and Alabama One Health Record to access and share your health information with other participants of these HIE’s for treatment purposes and for payment of treatment services. These HIE’s allow any health information organization that participates in the HIE’s to have secure electronic access to patients’ records.

You may opt out of the Health Information Exchange by doing one of the following:

1. Mail either the completed, signed, and dated Care Everywhere and HIE Opt Out / Opt In form, or a written request to opt out of the HIE, to the Southeast Health Medical Records Department.

2. Instead of mailing, you may also bring the completed form or written letter to the Southeast Health Medical Records Office located on the first floor.
