To ensure all patients receive a thorough evaluation and the best care in a timely manner, we kindly request that you bring the following on the day of your appointment:

• Insurance ID cards
• Identification card, driver’s license or government ID
• Co-payment or pre-payment
• Medication bottles or listing of current medications with dosing information
• When appropriate clinics may request additional information prior to the appointment

We kindly ask that new patients arrive 30 minutes prior to your appointment time and that established patients arrive 15 minutes prior.
