Southeast Health joins eight national healthcare leaders in Quality Improvement initiative

The MSQIC (Medical Staff Services Quality Improvement Committee) is pleased to announce the participation of Southeast Health in a collaborative nationwide mentored Quality Improvement initiative along with eight other top US Healthcare pioneers.

Facilitating Anticoagulation Safe Transition (FAST) project, originally initiated in Jefferson Health, that has now been launched through SHM (Society of Hospital Medicine) as a multi-center initiative aiming to reduce re-admission rate and complications (bleeding or re-occurrence of thrombosis) for patient admitted with Pulmonary Embolism (PE) or Deep Vein Thrombosis (DVT).

Southeast Health was selected among eight other top centers to collaborate in the study by implementing best practices to achieve this goal. A multi-disciplinary team of healthcare professionals at Southeast Health including Internal Medicine faculty:

  • Sagar Kulkarni, MD, Resident
  • Devesh Dahale, Director of Health System Engineering
  • Jacey Cox, Director of Pharmacy
  • Meagan Campbell, MSW, Case Management
  • Whittni Barrentine, Nurse Manager 2 East

The initiative will be implemented under the leadership of Arash Velayati, MD, and Ravitei Goteti, MD.

FAST attempts to use multiple measures, including evidence-based venous thromboembolism (VTE) treatment, careful medication reconciliation, patient education, meds to bed service, early post discharge follow-up calls, and early clinic visit to improve outcomes for patients with acute VTE admitted to Southeast Health. This implementation phase for the initiative begins in December 2020 and will be the first nationwide initiative of this kind that Southeast Health be involved in.
