To expedite the process, we kindly ask that you have the following information available at the time of your call:

• Patient demographic information
• Copy of insurance card(s)
• Reason for visit
• Supporting documentation
• Office Notes
• Lab/Test Results
• MRI Report
• X-Ray/Report
• Medications List

Note: Please share the 12 month Cascading Patient 1st for Alabama Medicaid patients.
